The ViewPoint FUSE implementation team worked with a healthcare provider to design and develop a distance care program for diabetes patients. The solution allowed diabetes patients to register and develop their care management plan online and report their progress and symptoms. Furthermore, the nurses assigned to the diabetes patients were able to monitor progress in real-time. The project included the following elements:
Customizable care management plans (Action Plan), each with their own set of configurable alerts and data to be collected;
Patient portal for patients to report their lab results, Quality of Life outcomes and questions to care providers;
Nurse portal for the real-time management of incoming lab values and questions;
Case tracking tools including "To Do" lists and correspondence tracking;
Patient data alert solution in real-time to provide quick response to patient events or questions; and
Management portal to view real-time blinded data aggregation describing patient demographic, lab and Quality of Life trends.
